These quotes represent two different approaches regarding the performance of fundoscopic examination prior to lumbar puncture in cases of suspected acute bacterial meningitis.


                                                       What is your approach?


Assuming that your patient is a 7 y/o previously healthy boy, and that this patient presented with 24 hours of fever, photophobia, and meningeal signs. He has no focal signs, no signs of septic shock, did not present with focal seizures or coma (GCS 15), and the platelets count is 350,000.  You intend to perform a Lumbar Puncture.

I will order a CT scan with contrast medium, and will perform Lumbar Puncture only if this scan rules out a space-occupying lesion.
